Navigate to the Incident Reports Via the Resident Record
1. Click on "Select Resident" under the "Resident" tab.
2. Select a client.
3. Click on "Incident Report" under the "Care" dropdown.
All incident reports for this client are displayed here.
4. Click on the "New Entry" button
5. Fill out the form.
Enter the staff member who reported the incident.
Enter the date and time of the incident.
Select the location of the incident. Multiple can be selected.
Select who was present at the time of the incident. Multiple can be selected.
Select the incident type. Multiple can be selected.
If the "Resident Injury" incident type is selected, a body map will appear. Select the location of the injury on the body map.
Enter notes or upload photos if necessary.
Click on the "Actions Taken" header to expand the section. Fill out the information as needed.
Click on the "Persons Notified" header to expand the section. If anyone such as a family member or a physician were notified about the incident, that information can be entered here.
When these signature buttons are selected, the manager can receive a notification requesting their signature. If neither button is selected, no notifications will be sent.
Navigate to the Incident Reports Via the Charting Page
1. Navigate to the Charting page
2. Click the "Incident" tile in the "Resident Warnings" module.
This will display all of the incidents for the entire home.
3. You can also click the "Incident" warning tile next to a clients record as seen below.
Create an Incident Report Via the Quick Tasks Module
1. Click on "Add New Incident" in the "Quick Tasks" module on the home page or Charting site.
2. Select the client for whom the incident will be recored. 3. Fill out the form. 4. Click the "Save" button to finish.
